Dr. Grumpy: "Any major illnesses in your family?"

Mr. Mendel: "My first cousin was adopted, so I have no idea what my aunt had."

It kinda makes sense. Since the cousin was adapted, this said individual was not able to observe any hereditary diseases in the blood relative i.e. child of his aunt. And if aunt is no longer alive and he does not talk to any surviving family members, then he truly would have no idea.
...Working with general public every "blank" day in the pharmacy...

Well, no brain points, but at least the patient knows that adoption means that there is no genetic connection to the cousin. (Who might be a walking, crawling, etc. encyclopedia of genetic disorders.) Which is why the patient is thinking of this cousin.
Yes, it is more profitable to talk of Mom and Dad and their sibs or the patient's sibs and their kids, but the cousin might be the only living relative and the patient is telling you that he/she does not know what the aunt might have had.
So, Dr. Ibee, I agree with another poster in another section of hte blog -- time for vacay! Curuising again?

my brother in law always worried he would die of a heart attack in his early 40's like his step dad did.we tried to explain to him that it didn't work like that,guess what he died from,yeah he was in his early 40's too.

This actually kind of makes sense?? Maybe he meant his aunt was unable to have kids so had to adopt and he isn't sure what made her unable to conceive?

Is he saying that he thinks his aunt may have been infertile, but does not know why?

I'm with the last two people who commented. I figured that the guy knew his aunt didn't have kids naturally so perhaps she had a medical problem.
