I am going out on a limb and think that this is not an issue that is
due to any condition they may or may not have? Men sometimes astound me with their thoughts on this subject. My husband after we were nearly married would wake me up because he thought I stopped breathing! I didn't, I just did not snore! Then after our first child was born he
did not want to hold him because he was afraid he would die. Well, I am
still here and our 28 yr old son is too. This kind of thinking seemed to be a trend amongst the men we knew. It did bring a laugh and eye roll so thank you!

That couple is obviously in need of some sort of objective third-party mediator on the issue. Perhaps you could refer them to Dr. Phil. Or maybe Dr. Magic8Ball, I heard he's easy to get into.
If his sister is as intelligent as him then I say the wifes sister should get them..

I am a lawyer and I get asked that question all the time. My response is always the same, choose the one with the least offensive substance abuse problem. Gives them something to reflect upon.
