I see this all the time. The thing is that I'm reading first/second year med student notes...(I volunteer at a med student run clinic.) The latest was:

A person?.......a tree?........how about an agitated barn animal?
Maybe there was an itinerant sneeze or missing punctuation? ...during the seizure, she was shaking in all limbs as well (Aaachhhooooo) ..as in all four extremities? Hmm. Aachhhooo!
