This is a perfect example of a Mail Merge not linked to a data base.

I'm more upset about the inconsistent variable naming. Why does only the doctor's first and last name get underscores while all other fields are run together?

Don't see a problem....it is Hippa compliant.
No confidential patient information has been exposed.

My bologna has a first name
It's <-B-F-I-R-S-T-N-A-M-E->
My bologna has a second name
It's <-B-S-E-C-O-N-D-N-A-M-E->
Oh, I love to eat it every day
And if you ask me why I'll say
'Cause has a way
With <-P-R-O-D-U-C-T->

Well...at least the quality parameter in some tracking system (that creates inaccurate ratings of performance of individuals, departments, companies) was ticked off as completed.

My mail order pharmacy always tells me that the price of my refills is not available when I place my refill requests - and my insurance only covers one PBM (has local and online divisions, names start with "C"). So I have no way to find out what a refill will cost me..until it is billed...and that information is not up on the web until the order is billed and shipped!

BUT their system does have the capacity to provide that information...just never to my household in past 3 years.

Data analytics and computer generated reports =fouling up communication and collaboration. The convergence of this 'new and improved' tool and HIPAA is ensuring that nobody can be certain that any single form means anything.
