Oh, you meant the concept of ER. See when I say "ER", I tend to think of a building with doctors and nurses and beds and such...
i didn't know you treated my older brother. very interesting :-).
I had one the other day that he couldn't "phantom (sic) the depths to which his cancer extended."

As a CNA/HHA, it seems like I have conversations like that ALL the time with my elderly clients AND their family "caregivers". Except less lofty-sounding, just more- WTF.

Never mind the conceptual stuff, stick with concrete, and go to ER when you have these symptoms...
