Makes about as much sense as ... anyone throwing around a verb, and a few nouns, with a preposition and a determiner to connect them all. Anyone up for a game of Mad Lib?

Sounds like a drop down form, with a text entry portion. Whoever did data entry may have accidentally chosen the wrong option from the drop down menu, and then added the text portion about how the patient was able to work in the garden with help.
