Yesterday a note came home from my son's school advertising a fundraiser type of thing where the teachers of the school are going to 'surprise' the students by working there for the evening. I can't figure out just how the students are going to be surprised when it is written out right there on the note.
I'm a transplant to the South (and some days I really miss Chicago) and THAT is a definite case of "Bless her heart" which covers poor taste, ugliness and stupidity. Or all three.
Wow...at least I'm sure not all of us in medical records are that out of it. I'd never write something like that on a request form; thanks for showing me I'm at least of reasonable intelligence!
I have this same thing happen at least twice a month.
I also have insurance companies who request medical records and leave out a company phone number/contact name, fax number, or mailing address. Then they get cranky when they call to check on the progress of the records and are told they've not yet been sent.
reminds me of the overhead pages we listen to all day " steve, if you are still in the store, please call ext 403..." um, ok. and if you aren't in the store..fuggetaboutit.
I work in an academic medical center, in an academic department, but somehow there are about 50 doctors' offices in town who have my voice extension as some other doctor's office fax. It's never for the same doctor twice. I get at least one a week.
I transfer them to our fax machine, then go to the fax and see who is being an idiot today. Sometimes I call them to tell them that we're not a medical office and they have just violated HIPAA and would they please use the correct fax number?
Can't tell you how many people ask me what the fax number is.
I had a credit card company send me a nasty letter about a late bill. I called them to say I had never recieved the bill, and when they checked their records it turned out the previous two bills had been returned with "No such street address" written across them. I asked why they had not phoned me since they had my number, and was told that they always contact people by mail about late payments.
I paid off that card and cancelled it ASAP after that. And they couldn't understand why...
Ranks up there with the VERY few angry VMs we get on our after hours line. It pages a pharmacist once they are done recording the VM.
"I NEED A PHONE CALL BACK ABOUT THIS IMMEDIATELY! MY LEFT TOE IS FALLING OFF AND I DON'T KNOW WHAT TO DO! PLEASE CALL ME BACK ASAP!"
No name nor phone number in message.
THEN -- we had one who called us the next day during our normal business hours SCREAMING at my staff about why we didn't call her back. I played the VM back for the person and she hung up...

We recently asked a local rest home to re-fax something. They said they couldn't because they had already faxed the original.

I've had similar exchanges via email with coworkers at the university where I work.
Coworker email to me: "I haven't received any responses to my emails this morning. I think my Outlook is acting up. Let me know if you get this."
My cheeky email reply back to coworker: "Sorry, I didn't receive your email. Try restarting your computer and send it again :-)"
Coworker email back to me after about 5 minutes: "OK, I've restarted everything. Let me know if you get this one."
Yes, I confirmed via telephone they were serious (face on desk).
