Why use your brain when you can buy an EMR and look like an idiot?

This is from a patient note that another doc faxed to my office this morning. The only things I've whited out listed the patient's pharmacies.

  2. As a medical transcriptionist, I cringe when I see these EMR entries. WTF? Oral and oral? Um...p.o. perhaps? It doesn't even specify the dosage or frequency!

  6. EMRs are always programmed by people who know nothing about medicine. It's totally counterintuitive. I ended up basically having to re-program the one at my last job because it never printed what the surgeons wanted it to say. If I saw this, though, it'd be way too tempting to program it to say Oral and Anal. ;-)

  7. Nystatin oral and oral? Forgive me if i am mistaken, but isn't that a "swish and swallow" med? (Or mouthwash type med?) And you take it........orally? Opposed to rectally? *scratching head*
