Whenever I see something like this, I only feel admiration that someone else was able to present a more professional statement than the one I use and it's probably safe to suggest they weren't able to come up with a definitive diagnosis right away. "More tests are necessary. We will try to look for a reasonable explanation, but it may take some time." They wish to leave final answer open until all possibilities are exhausted, up to and including exposure to moon dust, radon, perhaps someone consumed green apples, and in this particular and specific circumstance under highly unusual conditions the Krebs cycle went in reverse, possibly, unusual and unforeseen misalignment of the stars, though that makes it sound too much like voodoo medicine.
Sometimes it's nice to recall that we're all just heaps and heaps of electrical, magnetic, etc attractions among atomic particles.
For the truly pompous physician, listing a long differential is an opportunity to demonstrate the number of rare diseases you can name.
