I think you are afflicted with Hyper Observational Disorder, which is an extremely rare diagnosis in this day and age, it is characterized by being overly attentive that which is going on around oneself, a heightened sense of awareness to environment and is only cured when you recognize the need to stop paying attention along with the remainder of the human race.

  6. I suppose this is in an electronic charting system which has a lot of built-in boilerplate fields. It's understandable that a medical professional would want to speed through the tedious parts and concentrate on the substantive parts.

  7. In paper charting days, I once had a physician draw a tombstone saying RIP with an arrow saying PATIENT. I shit you not. No note..just a picture. I was amused and horrified. Thank God for EMRs.

  8. Eh, what was that about 'brevity being the soul of wit'? (At the time of the original quote, probably 'wit' was used, as opposed to 'witless' and not 'witty'.)

    As for the tombstone in the chart, sometimes 'horrified' is a good thing, such as pertains to those unfilled blanks on the EMR that provide simply no useful information at all.
