  8. Typical EMR note. The advent of electronic records has worsened care or at least the documentation of care more than anything else. You have prefilled blanks or blanks you check off and the computer writes the note. I suspect the doc just checked on the required box saying he was not suicidal and the computer wrote the note.

  10. I usually just put 'NA' or 'not applicable' in those blanks, but I wish I could just 'control' 'c' those lines of script and leave the pertinent details. Thank goodness I am usually in too much of a hurry to feel the invisible hand of Big Brother composing the consult note per the template.

  13. That's the trouble with electronic record-keeping ... it gives you a series of boxes to check that doesn't even apply to the situation, and won't let you complete the record without checking one. Leads to notes like this lol
