I got dragged in to see a hospital consult this morning. The admitting physician's note featured this:

    Notice the quotation marks. That's a direct patient quote. Not the sort of answer than gives the Doc much information.

    Looks like from the " " that this is what the patient or relative/companion told him and they could give no other info. Could it be a med (antiD?) for PMS?

    She is on a "bitch pill" - would have suggested a direct patient quote.

    "She is on a bitch pill" - definately makes it look like someone else is describing her ie. the doctor

    Either way, why would you write that down? Certainly could have been phrased better.
