Whoa, Nelly!

From another doctor's hospital dictation, listing the patient's pulse:

For my non-medical readers, a normal adult resting heart rate is 60-100. the above (I hope) is a typo.

  3. Funny how similar "eighty to ninety" and "eighty-two ninety" sound.

  6. That's a transcriptionist who wasn't thinking while listening. Or speech rec, which has no brain at all.

  7. Yeah, that looks like a transcribing error more than a typo.

  8. There needs to be some sort of lab value spell check.

  10. @Anonymous at 2:30: You're right; all the hallmarks of a Speech Wreck that went un-proofed.

@Rotating Intern--There is indeed a lab value spell checker: A competent and experienced editor/transcriptionist.

(As long as docs appear to think it's okay to let crap like this go on patients' charts without being proofed--because, after all, the vendor told them the software was 99.9 percent accurate--I'm gonna offer these gentle reminders that it just ain't so. It's kind of like having a free Risk Management consultant. Heh).

  11. @Horse on a Mattress - well, never have been a transcriptionist, I didn't want to be rude - but I completely agree with you.

  12. Grumpy,

You're a neurologist. Why are you concerned with the patient's pulse? I thought the heart only exists to pump blood to the brain, spinal cord and the synapses.

  13. And the heart sounds a lot like a drum roll.
